Hospital Corporate Name Approval

Corporate name approval by the State Board of Veterinary Medical Examiners (SBVME) must be sought under the following circumstances:

  1. If you are incorporating as a professional corporation or as a professional association; and
  2. A surname will not be used in the name of the hospital (e.g., Friendly Veterinary Clinic).

If 1 and 2 above apply, please print and complete the applicable form Application for Certificate of Authorization for Corporate Name of Professional Corporation and mail it to:

There is no fee associated with this request for approval. Once the request is received by the SBVME's office, it will copied and provided to the SBVME for review at its next regularly scheduled meeting. The SBVME meets once per month, usually the fourth Thursday of the month, except for months of November and December.

Once the SBVME approves the request, it will be forwarded to the Maryland Veterinary Medical Association (MVMA) for its review and approval. The MVMA will notify the Board in writing of its approval.

After the letter of approval is received by the SBVME, a Certificate of Corporate Name Approval is prepared by the SBVME and sent to the requestor.
